False PHD is a powerful passive item in The Binding of Isaac: Repentance that completely reverses the strategy behind taking pills. It turns what are usually negative items into massive offensive and defensive upgrades. You can find it in the Devil Room item pool. Core Effects

PHD / Lucky Foot / Virgo: If you hold one of these items alongside False PHD, the conversion mechanic is neutralized. Pills can appear as both good and bad again. However, swallowing a bad pill will still grant the +0.6 damage boost or a Black Heart.

Characters with Multipliers: Characters like Judas, Azazel, the Forgotten, or Tainted Lost benefit exponentially because their high built-in damage modifiers multiply the flat +0.6 damage increases.

Rock Bottom: This item prevents your stats from ever decreasing. If paired with False PHD, you gain the flat +0.6 damage bonus from Stat Down pills without actually losing any of your other stats.
